We are always happy to receive complaints and compliments to help us in the design and delivery of our services.
Positive feedback regarding recent interactions with the surgery are always gratefully received by our team and can be sent by email (email@example.com) or post.
If you wish to make a complaint, then we ask that you please fill out one of our complaint’s forms, you can do this using the form below, alternatively you can collect complaints form from the surgery. Alternatively, you can contact the Practice Manager by telephone or by letter. We will investigate and try to resolve your complaint as soon as we can. We will acknowledge complaints within three working days, you can complain up to 12 months after a concern.
