New Patient Registration

Surgery.jpgIf you live within our practice area you are welcome to register with us; please visit our Hope site to register. 

Click here to check if you are in our catchment area.

You must visit Hope practice in person where we will give you information on our practice and ask you to complete
a registration form and a new patient questionnaire.
You must bring with you some form of photographic identification such as a passport, driving licence, NHS card, and a recent utility bill.
We will ask you to make an appointment to see one of our nurses where we can ask you about your health, complete our records and answer any health related questions.Guide to GP Services.

How to Log In
New Users please read the following before attempting to register with this service
There are two registration screens.
Screen 1 details - Your Registration Information
It's very important that only genuine patients are granted internet access. The best way we can ensure this is to issue patients with a Registration Letter, which can only be obtained in person at your Practice. You cannot create an account without one.

The registration letter will give you your:
PIN number
Practice ID number
Access ID
NHS number

The information you enter on the first screen enables the system to check your identity when you log on.

Screen 2 details
This screen is for existing users to sign in, you will need:
Practice ID number
Access ID number
