During the Covid-19 pandemic we are not accepting paper registration forms.   When we are back to some kind of "normal" we will ask you to bring some form of photographic identification such as a passport, driving licence, NHS card and a recent utility bill to complete the process and sign the GMS1 form.
We will ask you to make a telephone consultation to speak to one of our GPs or nurses where we can ask you about your health, complete our records and answer any health related questions.

There are two registration screens.
Screen 1 details - Your Registration Information
It's very important that only genuine patients are granted internet access. The best way we can ensure this is to issue patients with a Registration Letter, which can obtained by telephoning the Practice. You cannot create an account without one.

The registration letter will give you your:
PIN number
Practice ID number
Access ID
NHS number

The information you enter on the first screen enables the system to check your identity when you log on.

Screen 2 details
This screen is for existing users to sign in. You will need:
Practice ID number
Access ID number

Named Accountable GP

Our new NHS Contract states that all patients must be allocated a named GP.
All current patients registered with this practice are allocated a named GP
Any new patients will be allocated a named GP when they register
Your named accountable GP is responsible for you overall care, however you are still welcome to see any GP within the Practice not necessarily your named GP.
If you would like to know the name of you accountable GP, please ask at reception.
